Southlake Sleepwell is the dental sleep medicine practice of James A. Holt, DDS. We provide custom oral appliance therapy for adults with diagnosed obstructive sleep apnea, chronic snoring, and CPAP intolerance. We do not diagnose sleep disorders. We work alongside the sleep physicians who do.

The result is co-managed care that respects the boundaries of each provider’s role: medical management and diagnostic follow-up stay with you, the appliance and the dental side stay with us, and communication flows clearly in both directions.

Co-managed care, not parallel care.
We rely on your diagnosis and your treatment plan. Sleep studies, follow-up sleep studies for verification, and ongoing medical management remain with you. We handle the appliance and the dental side.

Clear, timely communication.
You’ll receive a fitting confirmation when treatment begins, the appliance specifications, and an update at each titration and follow-up visit. If we identify clinical concerns during care, we contact you directly.

Patients who actually use their treatment.
The case for oral appliance therapy is largely a case for compliance. For patients who can’t or won’t tolerate CPAP, an appliance they actually wear beats a machine they don’t.

Ideal candidates for oral appliance therapy

The American Academy of Sleep Medicine recommends oral appliance therapy for:

  • Adults with mild to moderate obstructive sleep apnea who prefer an oral appliance to CPAP
  • Adults with severe OSA who cannot tolerate CPAP or refuse to use it
  • Adults with chronic primary snoring without OSA, after clinical evaluation, rule out underlying causes

Candidates should have adequate dentition to anchor the appliance and no active periodontal disease or significant temporomandibular joint disorders that would contraindicate treatment. We perform a brief dental evaluation as part of the initial consultation to confirm candidacy.

The referral process, step by step

1. Send the referral.
Include the diagnosis, the sleep study report or interpretation summary, and any specific appliance preference. Phone, fax, or secure email all work.

2. We contact the patient within one business day.
We schedule the fitting consultation directly. Patients without a confirmed diagnosis are referred back to you or to a partner sleep physician for evaluation first.

3. Fitting and titration over six to eight weeks.
Digital impressions, lab fabrication, fitting appointment, and a series of short titration visits to find the patient’s optimal setting.

4. Follow-up sleep study coordination.
Per AASM guidelines, we coordinate with your office on the verification of the sleep study once titration is complete.

5. Long-term care.
Annual appliance evaluation, refit or replacement when needed, and ongoing communication with your office about treatment status.

Oral Appliance Therapy

A small, custom-fit device worn at night. It holds the lower jaw slightly forward to keep your airway open. No mask, no hose, no machine noise. The standard of care for mild to moderate sleep apnea and a strong option for severe cases when CPAP fails.

CPAP Alternatives

If you've been diagnosed with sleep apnea and your CPAP is collecting dust, you're not alone. Roughly half of patients abandon their machines within a year. An oral appliance is often the next step.

Snoring Treatment

Loud, persistent snoring is more than a nuisance. It can signal an underlying airway issue, and it costs the person next to you their sleep too. A custom appliance can quiet most snoring overnight.

Sleep Apnea Treatment

Untreated sleep apnea raises your risk of high blood pressure, heart disease, stroke, and accidents. Treating it well can change how you feel every day. Dr. Holt's approach is co-managed with your sleep physician, from start to finish.
